Contest Forms have been updated to include an instructions page with mailing details, and now feature the Parental Consent form within the same PDF. While standalone Parental Consent forms remain available if needed, most entries will only require a single PDF instead of two.

NOTICE: We will be changing version numbers on all forms and assets starting in 2025 to an incremental # versioning system (increment#) rather than “year.increment#” to make it clearer to track changes and prevent confusion.
